Mirror Ritual: self-reporting questionnaire

Survey form used to allow users of "Mirror Ritual" to self-report on the experience of using an interactive affective interface. The survey contains 3 questions in each of the five components of meaningful experiences for HCI and UX as described in the proposed framework by Mekler and Hornbæk.
